Sculptra is often described alongside dermal fillers, but its mechanism is fundamentally different. Understanding that difference determines who it is right for.
Fillers versus biostimulators. A hyaluronic acid filler occupies space in tissue and gives immediate volume. The result is visible the moment it is injected. A biostimulator does not add lasting volume itself, instead, its microparticles trigger a controlled inflammatory response in the dermis that produces new type I collagen over three to six months.
What Sculptra is made of. Poly-L-lactic acid (PLLA), a biocompatible material that has been used in medical implants for decades. It is reconstituted with sterile water before injection and is fully absorbed by the body over 18 to 24 months. What remains is your own new collagen.
Who Sculptra suits. Clients in their 40s, 50s, and beyond who have lost volume across the mid-face, temples, cheeks, and jawline. Clients who want a gradual, natural-looking rejuvenation rather than an immediate change. Clients with mild to moderate skin laxity where volume restoration itself contributes to a lift.
The protocol. Typically two to three sessions spaced four to six weeks apart. Each session uses one to two vials. Full results are visible three to six months after the final session and last two to three years.
What to expect immediately after treatment. Some swelling and mild tenderness for 24 to 72 hours. We ask clients to massage the treated areas five minutes, five times a day, for five days after each session. This helps distribute the product evenly and reduces the risk of nodules.
Risks and how we manage them. Small palpable nodules are the most-discussed potential risk. Modern reconstitution protocols (higher dilution, longer hydration time) and diligent post-treatment massage reduce the risk considerably. We reconstitute at least 24 hours before treatment.
Who Sculptra does not suit. Clients wanting immediate volume are better served by hyaluronic acid filler. Clients with active infection, autoimmune conditions affecting the skin, or a history of keloid scarring need a careful risk assessment.
The honest read. Sculptra is one of the most rewarding treatments when the goal is quality rather than quantity, real collagen rebuilt over months, in the client's own tissue.
