Hyperhidrosis treatment uses small intradermal injections of botulinum toxin to switch off the overactive sweat glands in localised areas, most commonly the underarms, palms, and soles. By temporarily blocking the nerve signal that triggers the sweat glands, the treatment dramatically reduces excessive sweating without affecting body temperature regulation overall. It is one of the most reliable, evidence-based options for primary focal hyperhidrosis, with effects lasting several months per session. The procedure itself is quick, well tolerated, and allows an immediate return to normal activity.
Botulinum toxin is injected in a precise grid pattern across the affected area. The toxin blocks the release of acetylcholine at the nerve endings that supply the eccrine sweat glands, so the gland no longer receives the signal to produce sweat. The grid spacing is calibrated to cover the full sweating area, and the dose is adjusted to the location (underarms typically take a higher dose than palms or soles). Effects begin around day 4-7 and stabilise by 2 weeks.

Sensation varies by area. Underarm injections are generally well tolerated. Palms and soles are more sensitive, so a topical anaesthetic, ice, or vibration is used to make the treatment comfortable.
Compensatory sweating from a different area is uncommon with localised botulinum toxin treatment, unlike with surgical sympathectomy. The body's overall thermoregulation is unaffected.
